Update terraform-provider-ct plugin from v0.2.1 to v0.3.0

* Provide migration instructions for upgrading terraform-provider-ct
in-place for v1.12.2+ clusters
* Require switching from ~/.terraformrc to the Terraform third-party
plugins directory ~/.terraform.d/plugins/
* Require Container Linux 1688.5.3 or newer
